    Adult Protective Service Worker

    Temporary Part-Time Contract (28 hours/week) – May 2026 to May 2027

    Family Services Perth-Huron is a non-profit organization located in Stratford, Ontario, that provides counselling and developmental services for children, adults, and families. We are dedicated to the vision of “Our community strengthened one person, one family at a time.”

    Join our Adult Protective Service Worker (APSW) team, where the role of the APSW is to support adults with developmental disabilities living independently in the community, or with a desire to live independently in the community. APSW’s provide case management, service coordination, advocacy, supportive problem-solving, systems navigation, connection to resources and supports, etc., based on the individualized goals of each person supported. This existing APSW position provides services to Perth County and is based out of our Stratford office. This current employment opportunity is a part-time (28 hours/week) contract position (1 year) with possibility to become regular/permanent. The anticipated start date is May 2026.

    Job Summary:
    The APSW is accountable for establishing a voluntary working relationship with adults 18 years of age or older who are living on their own or are transitioning to live on their own in the community and have developmental disabilities. The APSW identifies and facilitates access to community supports as well as government-funded services and supports (e.g. Ontario Disability Support Program, Passport Program, Legal Aid, etc.). The primary focus is delivering case management support and advocacy services through regular face-to-face contact, fostering an environment where the individual supported has dignity, respect, choice and voice. Through a case management model, the APSW supports case coordination, planning, advocacy, access to community resources, team meetings, systems navigation (e.g. medical, legal, housing, financial, health and wellness, etc.)., promotes community awareness, and develops and maintains positive working relationships with community-based services to strengthen community approach to providing support.

    Qualifications:
    - Undergraduate degree in human services or a related discipline with 3-5 years experience supporting individuals with developmental disabilities.
    - Direct program delivery experience in the community social services sector, with a demonstrated working knowledge of community-based programs and related provincial and community support services.
    - The successful candidate must possess excellent written and oral communication skills, organizational skills, computer skills, maturity, tact, discretion, and the ability to analyze client and organizational needs.
    - The successful candidate must possess the ability to design and implement a service plan, have good interpersonal skills, and must have the ability to develop positive working relationships with individuals supported, colleagues, and community partners.

    Requirements:
    - Police Record Check (Vulnerable Sector Check)
    - Valid driver’s license with $2 million liability automobile insurance

    Pay: $27.00-$28.15/hour - 28 hours/week.

Self-Employed Independent Contractors are screened and matched to individuals and families seeking one-to-one support for a child or adult with a physical and/or developmental disability. Self-Employed Independent Contractors may provide community-based support, in home support/respite, and/or overnight respite.     Details:
    - Contracts vary based on the unique needs of the individual supported, with opportunities available to support children and/or adults receiving individualized funding for respite support.
    - Self-Employed Independent Contractors work closely with the person supported and their family/caregiver(s) to learn about unique, individualized support needs and person-centred goals.
    - Family Services Perth-Huron Coordinators provide support to families and Self-Employed Independent Contractors by offering resources, coordination of funding, and problem-solving.
    - Hours are flexible and worked out between the person supported and/or caregiver(s) and the Self-Employed Independent Contractor.
    - Daytime, evening and weekend opportunities available.

    Qualifications:
    - Related employment, life, or volunteer experience - Experience working with children and/or adults with disabilities, or a willingness to learn and gain experience - Comfortable operating as a Self-Employed Independent

    Contractor Requirements:
    - Police Record Check (Vulnerable Sector Check)
    - Valid driver’s license and access to reliable vehicle considered an asset

    Hanover and surrounding area.

    Home needed for a gentleman with a visual impairment who enjoys bowling, swimming, music, and the outdoors. He enjoys living in a family setting and being involved with his community.
